AT BIRTH OR AGE OF DIAGNOSIS

Determine eligibility for services with Help Me Grow and your county board. If your child is receiving services from Help Me Grow, ensure they have been transitioned to a new, open case with the county board when they turn three.

County board and/or Help Me Grow may provide services with both

local dollars and Medicaid.

TEEN YEARS

Support the individual to obtain a state ID.

Contact the Ohio Department of Developmental Disabilities

to explore employment opportunities.

Advocate for a Medicaid Waiver if you

have unmet needs.

Investigate SSI and prepare to apply.

At 16.5, begin thinking about guardianship or supported decision making.

At 17.5, contact attorney to discuss guardianship or alternatives.

Begin to consider transition to an adult healthcare provider (ask pediatrician for referral).

Begin discussion of transition plan.

AGE 18

If you haven't already, support the individual to obtain a state ID 

(can be obtained at any age.)

Apply for SSI one month after turning 18.

(Only the individual's income and assets

are counted after 18 years of age.)

21 YEARS OLD

If by age 21, the child is not receiving services through the county board, have the individual evaluated.

 

Consider transition from family home.

Investigate housing options.

Determine when parent should begin taking Social Security to maximize benefits for individual (Adult Disabled Child Benefits and Medicare.)

 

When parent/guardian turns

65 years of age (or before, if needed)

transition to other guardian or agent.
